The verses beginning with "Before I go now two fingers wide" do not make a great deal of sense literally. I believe that it is metaphor for the singer reminiscing about experiences in her life.
The Lahn is a river in Hessen, the state from which the band originates.

the expression "zwei Finger breit" (two fingers wide) refers to a hand gesture that in America is know as the "peace sign". It's a way of saying goodbye.

The refrain/chorus resembles a kind of a toast "to this, to that, to them".
"2 Finger breit" is a typical measure if you are buddies and don't have to make the effort to use glasses but drink off the bottle instead. A big sip.
I also don't know, if he's anxious. He's working hard to keep the relationship going, to make her feel well, to make her stay... some [earlier existing] ease is missing - 'strained', 'endeavoured' probably??
