The song title is a reference to Audrey Hepburn's Moon River from Breakfast at Tiffany's.

The broader story of the song is of a man who wants to recover from his alcoholism to be there for his wife and children, but struggles with his addiction and the uncertainty that his wife will wait for him to complete his journey. But, they are like the drifters in the song looking for "the rainbow's end" or his recovery, so it gives him the strength to continue on.
