This refers to imagery of an instrument that burns coal. But the burning hot coal is hidden from view. This is used toward people who seem innocent but are either sinister, sneaky about their intentions, or otherwise have some ability to hurt people. More frequently used towards females who appear to care about someone but are really just looking to use them for selfish gain.
